Media Purchase Request Form

Please fill out the form below for any multimedia that you would like to request
that the library purchase. If you are planning to use the material for a class,
please indicate the name of the class and when you need the material. We will
make every effort to get the item in a timely fashion, but allow as much lead time
as possible. The materials purchased for the Library will be cataloged and become
the property and responsibility of the Library. If you have any questions please
